Description
General Statement of Duties: The Nurse Manager for the Access (Admissions) team leads and oversees the Access (Admissions) Team, ensuring the efficient coordination of referrals and timely admissions across hospice, palliative, and in-home personal care services. This role is pivotal in driving admission growth, reducing time to admission, and improving referral conversion rates in alignment with strategic initiatives to diversify services and enhance patient access.
Level of Responsibility: Reports to Senior Nurse Manager. Supervises Access Team Specialists, Hospital Liaisons and Access RN’s
Responsibilities
Strategic and Clinical Leadership:
- Ensure rapid admission and onboarding of all eligible patients across hospice, palliative, and in-home personal care services.
- Identify and remove barriers in the referral-to-admission process to optimize access to care and support market competitiveness.
- Develop and execute departmental goals that align with strategic service line expansion and organizational performance objectives.
- Collaborate with the sales & business outreach manager to understand business development goals for the organization and the capacity requirements to support those goals.
Team Management and Development:
- Supervise, schedule, and support Access Team Specialists, Hospital Liaisons, and Access RNs.
- Foster a high-performance culture through coaching, mentoring, and performance evaluation.
- Maintain adequate staffing coverage and ensure the team is trained on eligibility, insurance, and new service lines.
Collaboration and Communication:
- Coordinate closely with the Palliative Care team, In-Home Personal Care services, and the Clinical Leadership team to ensure alignment and continuity of care.
- Partner with certifying physicians to help oversee eligibility determinations, providing narratives and confidence in admitting every eligible patient
- Act as a clinical and operational resource for internal teams and external referral sources.
- Work in partnership with the Marketing and Outreach departments to maintain strong referral relationships and promote organizational services.
Operational Oversight:
- Ensure compliance with federal, state, and organizational policies regarding patient admissions and eligibility.
- Complete administrative duties including scheduling, payroll approval, and performance documentation.
- Participate in rotating leadership on-call responsibilities, leadership development initiatives, and community engagement activities.
- Establish, measure and improve specific admissions-related KPIs and benchmarks
- Track and coach to productivity and conversion standards for all team members
- Evaluate and restructure scheduling processes to ensure an appropriate level of urgency in response to referrals, including weekends and after-hours
Requirements
Education & Experience:
- Current West Virginia eNLC Registered Nurse license is required.
- Demonstrated clinical knowledge related to hospice eligibility and payors.
- Minimum of two years’ nursing experience, inpatient preferred or hospice nursing preferred; five years of management experience required.
- Ability to establish and maintain good working relationships with the patient and family, the attending physician, coworkers and outside agencies.
- Computer proficiency is required.
- Provides own transportation, and a license to drive with current liability insurance. Driving record meets Hospice’s insurance company standards.
- Ability to multi-task and make informed decisions under pressure and time constraints.
Physical Qualifications:
- Ability to transport self to the office and patient care settings throughout the service area.
- Ability to walk from car to patient’s room in their care setting and perform essential job functions.
- Ability to lift and move patients and equipment.
- Ability to communicate verbally and in writing in English language.
- Ability to hear normal conversation in person and via phone.
- Ability to read normal or condensed print.
Equipment Used: telephone, computer, office equipment, medical equipment and supplies to provide patient care.
